Please bring the following when you come:
  • current year's tax forms and preparation booklet
  • copy of last year's income tax return(s)
  • W-2 forms from each employer
  • unemployment compensation statements
  • SSA-1099 form if you were paid Social Security benefits
  • All 1099 forms (1099-int, 1099-div, 1099-b, etc.
  • 1099-misc, showing any miscellaneous income
  • 1099 r form if you received a pension or annuity
  • All forms indicating federal income tax paid
  • dependent care provider information(name, employer, ID, Soc. Security number)
  • all receipts or canceled checks if itemizing deductions
  • Social Security cards or other official documentation for yourself and all dependents.
  • Those claining homestead credit should brong a rent certificate and or a copy of their 2009 property tax bill, and a record of any Wisconsin Works (w2) payments received in 2009/
  • Both spouses must be present to file a joint return.
 
Sunshine Supper provides and free and nutritious meal!
     In the spirit of hospitality and community outreach, the Sunshine supper provides a free and nutritious meal for people living in the Sun Prairie area....
     Our Values:
Hospitality:  All guests and volunteers who gather at the community supper feel welcome and comfortable.

Open door:  Meals are readily served to accommodate all individuals and families in a welcoming, friendly environment.

Community:  Meals are prepared together as a community of school, church, civic, and professional volunteers.  Volunteers eat alongside those in attendance to meet, greet and serve.

Service:  People from all walks of life are united in their desire to freely serve others.

     Plan Overview

     Once each week, a participating civic organization, church, professional or other group will provide the core components of a free and nutritious meal.  Extra, and unused food from restaurants, caterers and grocery stores will supplement the meal.

     One goal of the program is to provide a hot meal to individuals and families who would otherwise go without.  A second goal is to gather area residents around the dinner table to enjoy friendship and fellowship.  Meals will be served every Monday from 5:30-6:30PM in the lower level of the Masonic Lodge, 152 N. Bird Street.  Initially, the meal will serve 100 guests.

The Next Mobile Food Pantry Pick up is January 26,2010. ...If you have a prepicked number time is 5:00-6:00PM...At 6 PM others may come and be served!!  Bring identification and proof that you live in the Sun Prairie Area School District!  Mobile Food Pantry is the last Tuesday of each month.

Market Street Diner donates 10% of sales the first Tuesday of each Month.......Eat at Market Street Diner those Tuesdays...Thanks

Sun Prairie is a charming and thriving community that offers the appeal of small-town life within the bustling Madison metropolitan area. But Sun Prairie is experiencing growing pains. The social services that have always been here to support an increasing and somewhat hidden group of individuals and families in our community who need assistance are now at risk. It is time to create a new space to support our neighbors.

Sunshine Place Mission

The purpose of Sunshine Place will be to direct and administer the operation of a facility to house multiple charitable organizations providing social services needs to include but not be limited to food and clothing necessities in a manner that provides a single point of access to these services for the Sun Prairie community.
